Assertive Community Treatment (ACT)
Community-based treatment to help adults with a serious and persistent mental illness who have been diagnosed with a primary thought disorder (schizophrenia, schizoaffective disorder, major depressive disorder with psychotic features, or other psychotic disorder) with needs that exceed available care and result in frequent hospitalizations.
This “all-inclusive” mental health treatment model means you receive all mental health services through your team, including psychiatry, vocational, co-occurring disorder treatment, individual or group therapy, peer support services, psychiatric nursing, case management, and 24/7 crisis services. Comprehensive services can include planning and monitoring treatment, accompanying clients to doctor and dental appointments, assisting with managing benefits, and teaching daily living skills such as cooking, housekeeping, handling money, and finding transportation. Other interventions can include assisting with ordering, managing, and administering medications, supported employment, co-occurring disorders treatment, and psychoeducation for clients and their families.
