In 2021, CMMHC became a Certified Community Behavioral Health Clinic (CCBHC), an accreditation and model of care spreading across the US with positive results for hundreds of thousands of people. This enables us to be a “one-stop-shop” for individuals with mental and chemical health issues by providing comprehensive, integrated services to children and adults and meeting the overall health needs of every client (improving both physical AND mental health). Coordination and partnerships occur among team members, including multiple providers, social service agencies, counties, and other stakeholders, to increase the likelihood that a person receives care before they are in crisis.
CCBHC service delivery focuses on:
  • Clients accessing a wide array of mental, physical, and chemical health services
  • Intentional communication and coordination among all providers working with common clients
  • Person-centered care that respects the unique needs of individuals and families
  • A team-based approach in developing treatment or recovery plan goals
  • Utilizing evidence-based best practices to improve overall health outcomes
This innovative care model allows our community to access and receive coordinated care in the most appropriate settings, helps to improve outcomes, and saves money by diverting care from emergency rooms and inpatient beds.
