Mental Health Practitioner/LADC- ACT Co-Occurring Disorder Specialist

Central MN Mental Health Center has an opportunity for a Full-Time Mental Health Practitioner: ACT Co-Occurring Disorder Specialist/ Licensed Alcohol and Drug Counselor in our Monticello Assertive Community Treatment (ACT) program. The ACT Mental Health Practitioner is an active member of an evidenced-based clinical team, providing medically –necessary treatment, rehabilitation, and support services to identified recipients with severe and persistent mental illness (SPMI). The Co-Occurring Disorder Specialist provides Integrated Dual Disorders Treatment to ACT clients who have a substance use problem. The Duties may include but are not limited to:

  • Provides or facilitates the provision of co-occurring disorder treatment to clients
  • Serves as a consultant and educator to fellow ACT team members on co-occurring disorders
  • Implement services in the treatment plan for each recipient under clinical supervision
  • Complete assessments and maintain client documentation
  • Coordinate services with other programs and providers

The wage range for this position is $22.00-$26.40. The starting salary for each application will be based on years of experience for the position.

Qualifications

  • The Mental Health Practitioner shall meet the qualifications defined in Minnesota Statue 245.462 subdivision 17
  • Be a full-time equivalent co-occurring disorder specialist who has received specific training on co-occurring disorders that is consistent with national evidence-based practices. The training must include practical knowledge of common substances and how they affect mental illnesses, the ability to assess substance use disorders and the client’s stage of treatment, motivational interviewing, and skills necessary to provide counseling to clients at all different stages of change and treatment. The co-occurring disorder specialist may also be an individual who is a licensed alcohol and drug counselor as described in section 148F.01, subdivision 5, or a counselor who otherwise meets the training, experience, and other requirements in section 245G.11, subdivision
  • Clear DHS background study and sexual exploitations screening

Mental Health Practitioner: RN- ACT

Central MN Mental Health Center has an opportunity in our Monticello Assertive Community Treatment Team (ACT) for a full-time Mental Health Practitioner Registered Nurse (RN). The ACT: RN is an active member of an evidenced-based clinical team, providing medically necessary treatment, rehabilitation, and support services to identified recipients with severe and persistent mental illness (SPMI).  This position is responsible for managing the medication system; administering and documenting medication treatment in collaboration with the Psychiatrist, Nurse Practitioner (NP) or Certified Nurse Specialist (CNS). The duties may include but are not limited to:

  • Coordination and assessment of the recipient’s medication management plan so that optimal health is maintained.
  • Implement services in the treatment plan for each recipient under clinical supervision.
  • Complete assessments and maintain client documentation.
  • Coordinate services with other programs and providers.
  • Administrative and professional responsibilities.

This position would include some evening and weekend shifts and an on-call rotation.

Qualifications

  • Licensed by the State of Minnesota as a Registered Nurse.
  • The Mental Health Practitioner shall meet the qualifications defined in Minnesota Statute 245.462 subdivision 17.
  • Experience and training in behavioral learning techniques, rehabilitation theory and practice.
  • Complete initial and ongoing training requirements defined for the position to meet required standards.

Mental Health Practitioner: LPN- ACT

Central MN Mental Health Center has an opportunity for a Full-Time Mental Health Practitioner: LPN on our Monticello Assertive Community Treatment (ACT) team. The License Practical Nurse (LPN) is an active member of an evidenced-based clinical team, providing medically necessary treatment, rehabilitation, and support services to identified clients with severe and persistent mental illness (SPMI).  The LPN is responsible for managing the medications system; administering and documenting medication treatment in collaboration and under supervision with the Psychiatrist, Nurse Practitioner (NP) or Certified Nurse Specialist (CNS) and the ACT Registered Nurse. The Duties may include but are not limited to:

  • Instruct, assist, and support clients in areas such as; interpersonal communication skills, community resource utilization and integration skills, crisis assistance, relapse prevention skills, health care directives, budgeting and shopping skills, healthy lifestyle skills and practices, cooking and nutrition skills, transportation skills, mental illness symptom management skills, household management skills, employment-related skills, and integration into community living. Implement services in the treatment plan for each recipient under clinical supervision
  • Make changes in the Individual Treatment Plan (ITP) to ensure immediate and appropriate interventions are provided in response to changes in mental status/symptoms or behavior which may put the client at risk.
  • Drive to client homes or various locations within the community where services are provided, and transport clients.
  • Establish therapeutic relationships with assigned clients and meet with each client as planned.
  • Provide on-call services as scheduled.
  • Under the direction of the prescriber and in collaboration with the other nurses on the team, develop, revise, maintain and ensure adherence to medication policies and procedures, including transcribing, administering, and recording medications prescribed.
  • Assignment of medication administration including transcription of medication orders onto the Medication Administration Record (MAR); ordering medication from pharmacies; administering oral and injectable medications; and documenting medications administered into the MAR.
  • Initial and ongoing evaluation, with documentation of effectiveness, client’s reactions, receptiveness, and compliance to the medications, as well as complications and side effects of psychotropic medications prescribed.

The wage range for this position is $19.50-$23.40. The starting salary for each application will be based on years of experience for the position.

Qualifications

  • The Licensed Practical Nurse shall meet the qualifications defined in Minnesota Statute 245.462 subdivision 17 or Minnesota Statute 256B.0623 subdivision 5.
  • The Mental Health Practitioner shall meet the qualifications defined in Minnesota Statute 245.462 subdivision 17.

Mental Health Professional: ACT Assistant Team Lead- Monticello, MN

Central MN Mental Health Center has an opportunity in our Assertive Community Treatment (ACT) program for a Full-Time Assistant Team Lead in our Monticello office. The Assistant Team Lead is an active member of an evidence-based clinical team, providing medically necessary treatment rehabilitation, and support services to identified recipients with severe and persistent mental illness (SPMI). The job duties include but are not limited to:

  • Implement services in the Treatment Plan for each recipient under clinical supervision
  • Provides Leadership to ensure compliance with ACT standards as outlined in Tool for Measurement of ACT (TMACT).
  • Under the direction of the psychiatrist and Registered Nurse (R.N.), provides medication monitoring and education regarding health maintenance to encourage a healthy lifestyle.
  • Partner with Team Leader to assist in organizational duties of the day-to-day operations of team functioning and client service delivery
  • When clinically indicated, provides individual, group, and family therapy.
  • Makes changes in the Individual Treatment Plan to ensure immediate and appropriate interventions are provided in response to changes in mental status/symptoms or behavior.
  • Assists with referrals and screenings.
  • Participates in all mandatory team meetings.

The starting salary for each application will be based on degree level, licensure, and years of experience for the position. The wage range for an Unlicensed/Clinical Trainee is $24.7356-$29.6827. The wage range for a Mental Health Professional is $57,000-$74,400.

Qualifications

  • Master’s Degree in Psychology, Social Work, or Marriage and Family Therapy.
  • Must be a licensed mental health professional who is qualified under Minnesota Rules, part 9505.0371, subpart 5, item A; Or a Clinical Trainee working towards licensure as an LP, LPCC, LICSW or LMFT.
  • Must have a clear understanding of the ACT service delivery model.
  • Supervisory, program management, and leadership experience is preferred.

Early Childhood Mental Health Professional- Monticello

Central MN Mental Health Center has an opportunity in our Monticello Outpatient Mental Health (OPMH) Department for a Full-Time Children’s Mental Health Professional/Outpatient Therapist, primarily working with the 0-5 population.  We are looking for a dedicated and experienced individual who has a passion for person-centered, recovery-based mental health work with those experiencing significant mental health and co-occurring disorders.  The Outpatient Therapist is responsible for providing mental health services within a trauma informed framework.  The duties may include, but are not limited to:

  • Consistently demonstrate clinical competence and professionalism by effectively providing mental health therapy and diagnostic services to individuals and families in an outpatient setting.
  • Respond appropriately to crisis situations and rapid screen needs of clients.
  • Collaborate with social workers, outside agencies, and other care providers to provide integrated care to clients.
  • Opportunity for free evidence-based training in Child Parent Psychotherapy (CPP) and Attachment Biobehavioral Catch-up (ABC) is offered.

The starting salary for each application will be based on degree level, licensure, and years of experience for the position. The wage range for a Mental Health Professional is $57,000-$74,400. The wage range for a Mental Health Professional – Medicare is $60,000-$95,000.
