ACT Team Lead: 

Central MN Mental Health Center has an opportunity for a Full-Time Assertive Community Treatment (ACT) Team Lead in our Monticello ACT Team. The ACT Team Lead establishes, administers, and directs the ACT team, a self-contained clinical team which assumes responsibility for directly providing needed treatments, rehabilitation, and support services to identified clients with severe and persistent mental illness; supervises and evaluates the multidisciplinary team to ensure services are provided in a trauma-informed, recovery-orientated framework; and function as a practicing clinician on the team.

Qualifications

  • Possession of a Minnesota State license as an LICSW, LP, LMFT, RN, or LPC.
  • Three or more years of post-graduate clinical service with adults who have severe mental illness, within a mental health setting, community mental health service is preferred
  • Supervisory/program management experience preferred
  • Good organizational skills are a must
  • Clear DHS background study and sexual exploitations screening
  • Valid Driver’s License and acceptable driving record.

Registered Nurse (RN): ACT: 

Central MN Mental Health Center has an opportunity in our Monticello Assertive Community Treatment (ACT) for a Full-Time Registered Nurse (RN). The RN is responsible for managing the medications system; administering and documenting medication treatment in collaboration with the Psychiatrist, Nurse Practitioner (NP) or Certified Nurse Specialist (CNS). The Duties may include but are not limited to:

  • Coordination and assessment of the recipient’s medication management plan so that optimal health is maintained.
  • Implement services in the treatment plan for each recipient under clinical supervision.
  • Complete assessments and maintain client documentation.
  • Coordinate services with other programs and providers.
  • Administrative and professional responsibilities.

This position would include some evening and weekend shifts and an on-call rotation.

Qualifications

  • Licensed by the State of Minnesota as a Registered Nurse.
  • Experience and training in behavioral learning techniques, rehabilitation theory and practice
  • Complete initial and ongoing training requirements defined for the position to meet required standards.
  • Clear DHS background study and sexual exploitations screening
  • Valid Driver’s License and acceptable driving record.

Licensed Alcohol and Drug Counselor (LADC): Float: 

Central MN Mental Health Center has an opportunity for a Full-Time Licensed Alcohol and Drug Counselor (LADC) Float in our Outpatient Chemical Dependency programs. The LADC Float Counselor serves as a primary resource for the clinical team across the three outpatient chemical health sites, including Monticello, Buffalo, and St. Cloud. This position will office out of the Monticello or Buffalo location.  Mileage reimbursement to the other sites is included.

The Float LADC will work with each counselor to become acclimated to the various sites and groups offered, and will provide coverage in absences, planned and unplanned. This position will provide group coverage and case management as required.  Chemical health evaluations will also be a primary duty of this position.

DUTIES AND RESPONSIBILITIES:

  • Facilitate primary groups, lectures, experiential groups, and educational groups.
  • Support Primary Counselors by assisting with case management and treatment coordination.
  • Cover caseloads or perform intakes as requested.
  • Complete chemical health assessments
  • Continue to upgrade skills via training and supervision.
  • Maintain professional relationship with clients while following a strict code of ethics.
  • Perform other duties as assigned.

Looking for an individual who likes diversity.  Organizational skills are a must.  Ability to be adaptable to change in a fast pace environment is critical.  Strong group facilitation and engagement skills are important.

Possibility of a four day work week.  Hours will vary depending on program needs.

Qualifications

  • Bachelor’s degree in chemical dependency or related human services field, or exempt under MN statutes 148C
  • Certification as a Licensed Alcohol and Drug Counselor (LADC) through the Minnesota Department of Human Services
  • Counselors providing adolescent services must meet 30 hours of classroom instruction training requirements or training will be provided
  • Two consecutive years free of chemical use problems
  • Clear DHS background study and sexual exploitations screening
  • Valid Driver’s License and acceptable driving record.
  • Maintain current CPR certification
