Mental Health Practitioner: Targeted Case Management – Adult 

Our Targeted Case Management (TCM) team is expanding. Our program provides a passion driven, client-centered work environment with a unique blend of independence and team support.

Central MN Mental Health Center has opportunities for full time Adult Targeted Case Management positions. TCM provides home and community based mental health services to adults with serious and persistent mental illness (SPMI). The duties may include, but are not limited to:

  • Be familiar with the implementation of Health Integration, including care coordination.
  • Establish relationships with assigned clients and their supports, while addressing goals defined in their treatment plan.
  • Assess, refer, coordinate, and monitor services.
  • Communicate regularly with all involved providers involved to ensure appropriate care, reduce duplication of services and promote client autonomy.

Wage is dependent on experience and paid on an hourly basis.

Qualifications

  • The Targeted Case Manager shall meet the following qualifications defined further in Minnesota Statute 245.462 Subdivision 4:
    • Bachelors degree in one of the behavioral sciences or related fields from an accredited college or university
    • 2000 hours experience working with adults with serious mental illness and/or with children with severe emotional disturbances
  • Ability to work independently and as part of a team
  • Clear DHS background study and sexual exploitations screening
  • Valid Driver’s License and acceptable driving record.

Mental Health Practitioner: ARMHS 

Central MN Mental Health Center has an opportunity for a Full-Time Mental Health Practitioner in our St. Cloud Adult Rehabilitative Mental Health Services (ARMHS) program. The ARMHS provider is responsible for home- and community-based mental health services to individuals diagnosed with a mental illness and co-occurring disorders. The Duties may include but are not limited to:

  • Implement services in the treatment plan for each recipient under the clinical supervision of a Mental Health Professional
  • Complete assessments and maintain client documentation under the direction of the Mental Health Professional
  • Coordinate services with other programs and providers
  • Administrative responsibilities

Qualifications

  • The Mental Health Practitioner shall meet the qualifications defined in Minnesota Statue 245.462 subdivision 17
  • Experience and training in behavioral learning techniques, rehabilitation theory and practice
  • Clear DHS background study and sexual exploitations screening
  • Valid Driver’s License and acceptable driving record.
  • Maintain current CPR Certification

Mental Health Practitioner Care Coordinator – IRTS

If so, Central Minnesota Mental Health Center (CMMHC) is a great place for you to begin or grow your career working with those diagnosed with a mental health disorder. We have opportunities to support clients with complex diagnoses and co-occurring disorders as a Mental Health Practitioner Care Coordinator in our Northway Intensive Residential Treatment Services (IRTS) program. CMMHC provides intensive residential treatment services to individuals who are experiencing significant difficulty with daily life activities as a result of a serious mental illness.

Since we support programs on a 24/7 basis, we have a variety of shifts and schedules available:

  • Full-time
  • Days with every other weekend involvement. This position requires scheduling flexibility that may include evenings and weekend shifts

Our Mental Health Practitioner Care Coordinator responsibilities include:

  • Provide medically necessary rehabilitation services and adult rehabilitative mental health services
  • Direct and oversee the development of assessments and individual treatment plans with clients
  • Coordinate services with community providers/services

Required Qualifications:

  • Has at least 4,000 hours of supervised experience in the delivery of services to adults or children with

– mental illness, substance use disorder, or emotional disturbance; or

– Traumatic brain injury or developmental disabilities and completes training on mental illness, recovery from mental illness, mental health de-escalation techniques, co-occurring mental illness and substance abuse, and psychotropic medications and side effects;

Or

  • Holds a master’s degree or other graduate degree in behavioral sciences or related fields

Or

  • Holds a bachelor’s degree in behavioral sciences or related fields and completes a practicum or internship that (1) requires direct interaction with adults or children served, and (2) is focused on behavioral sciences or related fields

And

  • Clear DHS background study and sexual exploitations screening
  • Valid Driver’s license and acceptable driving record.
  • Maintain First Aid and CPR Certification

Preferred Qualifications:

  • Experience and training in behavioral learning techniques, rehabilitation theory and practice
  • Experience with facilitation of educational groups

Mental Health Professional 

Central MN Mental Health Center has an opportunity in our St. Cloud Outpatient Mental Health (OPMH) Department for a Full-Time Mental Health Professional. The Outpatient Therapist is responsible for treating clients with emotional, behavioral and mental disorders within a family driven and trauma informed framework.  The Duties may include but are not limited to:

  • Provide therapy to individuals, family and groups in an out-patient setting
  • Perform diagnostic assessments to determine a recipient’s eligibility for mental health services
  • Respond appropriately to crisis situations and rapid screen needs of clients

Qualifications

  • Master’s or Doctoral Degree in Psychology, Social Work, or Marriage and Family Therapy
  • License as an LP, LPCC, LICSW or LMFT
  • Two years’ experience in out-patient therapy is preferred
  • Clear DHS background study and sexual exploitations screening
  • Valid Driver’s License and acceptable driving record.

Day Treatment Program Manager: 

Central MN Mental Health Center has an opportunity for a Full-Time Program Manager in our Day Treatment program. The Program Manager provides oversight, direction, leadership, and clinical supervision to the Day Treatment Program and staff within a client driven and trauma informed framework. The Program Manager’s time is dedicated at least 50% to the supervision of the Day Treatment Program and staff and 50% to providing direct client services as a Day Treatment Therapist. The Duties may include but are not limited to:

  • Facilitate or Co-facilitate group and individual psychotherapy
  • Performs diagnostic and/or other assessments to determine recipient’s eligibility for services
  • Responds appropriately to crisis situations and rapid screen needs of clients
  • Oversees program budget, provides clinical and direct supervision of Day Treatment staff, Interviews, Hires and trains staff

Application review would begin in 3-6 months, after it is determined if the position is needing permanent placement

Qualifications

  • Master’s Degree in Psychology, Social Work, or Marriage and Family Therapy
  • MN license as an LP, LPCC, LICSW or LMFT, or working towards licensure
  • Two years experience working in Adult Day Treatment is preferred.
  • Must be a Qualified Clinical Supervisor

Licensed Practical Nurse(LPN): Northway IRTS: 

Central MN Mental Health Center has an opportunity for a Full-Time Mental Health Practitioner: Licensed Practical Nurse (LPN) in our Northway Intensive Residential Treatment Services (IRTS) program. The Licensed Practical Nurse (LPN) will provide direct medical and health care services to recipients who are experiencing significant difficulty with daily life activities as a result of a serious mental illness in a comprehensive 24 hour service. This position functions as a member of the IRTS treatment team and will include mental health practitioner tasks.  The Duties may include but are not limited to:

  • Provide medically necessary rehabilitation services and adult rehabilitative mental health services
  • Perform assessments and ongoing monitoring of health and medical needs of recipients as per directed by the designated RN. All services will incorporate care for individuals through nursing standards recognized by the board of nursing at the direction of an RN or licensed health care provider
  • Provide referrals and coordination with community providers to help the recipients be successful in the community

This position will work evenings and every other weekend.

Qualifications

  • Licensed by the State of Minnesota as a Practical Nurse
  • The Licensed Practical Nurse shall meet the qualifications defined in Minnesota Statute 245.462 subdivision 17 or Minnesota Statute 256B.0623 subdivision 5
  • Experience and training in behavioral learning techniques, rehabilitation theory and practice
  • Clear DHS background study and sexual exploitations screening
  • Valid Driver’s License and acceptable driving record
  • Maintains First Aid and CPR certification.

Part-Time

Licensed Alcohol and Drug Counselor: 

Central MN Mental Health Center has an opportunity for a Part-Time Licensed Alcohol and Drug Counselor in our St. Cloud Detox program. The LADC performs chemical dependency evaluations for clients, develops individual intervention, treatment, and recovery plans, and facilitates or leads group sessions. The Duties may include but are not limited to:

  • Provide clinical screening to admitted clients, following Rule 32 statutes
  • Complete or provide referral for chemical use assessment
  • Provide appropriate referrals for services based on screening and/or assessment
  • Provide client education as outlined in Rule 32 9530.6530, Subp. 4
  • Provide individual and group support to clients

This position will be every other Saturday for 5 hours. Start time is negotiable.  Great opportunity for supplemental income.  Possibility for additional hours or full time employment in the future.

Qualifications

  • Bachelor’s degree in chemical dependency or related human services field, or exempt under MN Statutes 148C
  • Certification as a Licensed Alcohol and Drug counselor (LADC) through the Minnesota Department of Human Services
  • Two consecutive years free of chemical use problems;
  • Clear DHS background study and sexual exploitations screening.
  • Maintain current CPR certification
  • Maintain professional competencies and appropriate licensure; continued education as required by the Board of behavioral Health
  • Valid Driver’s License and acceptable driving record.

Registered Nurse (RN) or Licensed Practical Nurse (LPN): Detox: 

Central MN Mental Health Center has an opportunity for a Part Time Registered Nurse (RN) or Licensed Practical Nurse (LPN) in our Detox Program. The nurse is responsible for providing and coordinating nursing care for detox clients within a family driven, trauma informed framework. The Duties may include but are not limited to:

  • Perform assessments of the client’s health, risk of self harm, monitoring for safe detoxification and monitoring for withdrawal symptoms; take vital signs including Oximeter.
  • Admit, transfer, discharge and coordinate the treatment plan of clients
  • Administer medications and treatments as ordered by the consulting physician
  • Develop care plans and individual abuse prevention plans for clients

This position would include overnight and weekend hours. Position would consider newly graduated nurses.

Qualifications

  • 2 year nursing degree from an accredited professional nursing program or certificate of completion from a 12 month accredited practical nursing program;
  • Registered Nurse or Licensed Practical Nurse in the state of Minnesota without encumbrance
  • 2 years’ nursing experience preferred
  • DHS Requirements of 2 years freedom from chemical use problems and signed attestation statement
  • Clear DHS background study and sexual exploitations screening
