Focus XII Transitional Housing

A low-intensity residential program for men transitioning to the community who have completed a treatment or incarceration program addressing substance use disorders or have had periods of sobriety at alternative levels of care and are returning to residential treatment services.

This step in the continuum of care is apartment-style living that helps you transition from a highly structured environment to an independent living community. The stay is typically 90 days, depending on your progress. Treatment methods include Cognitive Behavioral Therapy, psychoeducation, skills development, health and wellness, co-occurring disorders, emotional regulation, healthy relationships, and relapse prevention. There is an emphasis on collaboration with community services to encourage independent living skills. You can access group, individual, certified peer support, and 12-step support groups.
