Targeted Case Management
Adult and Children’s Mental Health Targeted Case Management (MHTCM) serves adults with serious and persistent mental illness and children with severe emotional disturbance. Clients who qualify for this program need to meet the requirements for Minnesota Rule 79 case management services and be Prepaid Medical Assistance Program (PMAP) recipients who live in Benton, Stearns, Sherburne, or Wright County. At CMMHC, our clients receive specialized services tailored to meet their specific needs.

Rehabilitative Services
The rehabilitation services from the providers here at CMMHC include assertive community treatment, adult rehabilitative mental health services, a community support program, adult day treatment, and intensive residential treatment services.

Crisis Services
CMMHC offers mental health crisis services to those having difficulty coping or adjusting to the everyday stresses of life.

Outpatient Services
Clients receive support and treatment for many life situations that include depression & mood disorders, relationship challenges, life changes, grief & loss, conflict resolution, and other struggles through individual or group therapy.

Chemical Health Services
CMMHC provides services to adolescents and adults with a substance use disorder or co-occurring substance use and mental health issues. Our professional staff assesses individuals with problems resulting from alcohol or drug use and provides treatment recommendations.

Children and Family Services
CMMHC focuses on helping children and families cope with mental health challenges to manage their lives more effectively. By teaching skills and providing education on dealing with problems, we help to foster the bond between parent and child.
