Adult Intensive Outpatient Substance Use Disorder Programming and Prevention 4 Teens (P4T) fall under Outpatient Chemical Dependency.
Outpatient SUD provides services to improve quality of life by delivering integrated substance abuse and mental health services. Treatment addresses mental health and substance abuse through collaboration and consultation with the goal of recovery – customizing supports to the circumstances and background of each individual. A multi-disciplinary team works with clients and encourages family involvement.
The MN Department of Human Services licenses CMMHC as an Outpatient SUD Treatment Services provider to adults and adolescents with both chemical and mental health disorders. Our services for adults are available in the Buffalo, Monticello, and St. Cloud offices, with additional programming specific to adolescents in the Monticello office.
Ongoing services within SUD include:
Adult Intensive Outpatient Substance Use Disorder Programming
This outpatient group for adults offers nine hours of treatment per week (for 10-12 weeks) depending on an individual’s needs and participation. Groups meet three hours daily, three days a week for the first six to seven weeks and two days a week for the remaining three to five weeks. Clients can then transition to outpatient or aftercare groups for continuing care needs. This service is available to adults at our Buffalo, Monticello, and St. Cloud offices.
Prevention 4 Teens (P4T)
This education-based substance use prevention class is for youth ages 12-18 experiencing problems associated with alcohol or other drug use but do not meet the diagnostic criteria for substance use disorder or more formal interventions. P4T provides education, support, and guidance for teens and their parents through two sessions with a counselor. The fee is $100, and clients must pay in full before the first class.
P4T is available in our Buffalo and Monticello offices.

Teresa received her associate’s degree in Addiction Counseling from Minneapolis Community and Technical College. She is a Licensed Alcohol and Drug Counselor (LADC). Teresa worked at Maple Lake Recovery Center and Hazelden Center for Youth and Family before joining the CMMHC team in 2012 as a Chemical Dependency Counselor.

Before joining CMMHC, Lily worked in child care for five years before becoming a Certified Pharmacy Technician. She is currently a Licensed Alcohol and Drug Counselor (LADC) based out of our Buffalo office.

Catherine received her bachelor’s degree in Psychology from Argosy University with a concentration in chemical dependency. She is a Licensed Alcohol and Drug Counselor (LADC), the supervisor for the Outpatient Chemical Dependency Treatment program, and the primary counselor for the morning Adult Chemical Dependency Treatment Group at our Monticello office.
