Targeted Case Management

You may also see this referred to as Adult Mental Health Targeted Case Management (AMH-TCM) or Children’s Mental Health Targeted Case Management (CMH-TCM)

Targeted Case Management supports adults with serious and persistent mental illness and children with severe emotional disturbance, working collaboratively with individuals and their families to ensure they receive tailored care to meet their needs.

These services include developing a functional assessment and individual community support plan, assisting clients with access to mental health services, referring additional supportive services as necessary, and monitoring service delivery.

Case Managers provide guidance and support to help individuals access medical, social, educational, vocational, financial, and other services related to improving their mental health.

Qualifications for Services

To qualify for TCM, individuals must be a recipient of a Prepaid Medical Assistance Program (PMAP) and meet the requirements for Minnesota Rule 79 case management services. They must also live within Benton, Stearns, Sherburne, or Wright County.

Processing of referrals occurs within 10 working days of receiving a current diagnostic assessment.

Dawn Benson-Hance

MS, LPCC, LSW

Dawn completed her bachelor’s degree in social work from St. Cloud State University and her master’s degree in mental health counseling from Walden University. She is a licensed professional Clinical Counselor and a licensed Social Worker. Dawn has been in the human services field since 1997 before joining the CMMHC team in 2007. She is currently an Outpatient Therapist and the Clinical Supervisor for the Adult TCM team. Her expertise include individual therapy for adults with mood and anxiety disorders, trauma with the use of eye movement desensitization and reprocessing (EMDR) related interventions and interpersonal relationship issues.
