Targeted Case Management

You may also see this referred to as Adult Mental Health Targeted Case Management (AMH-TCM) or Children’s Mental Health Targeted Case Management (CMH-TCM)

Targeted Case Management supports adults with serious and persistent mental illness and children with severe emotional disturbance, working collaboratively with individuals and their families to ensure they receive tailored care to meet their needs.

Targeted Case Managers provide guidance and support to help individuals access medical, social, educational, vocational, financial, and other services to improve their mental health. They assist with referrals to resources, services, and community supports to promote recovery and help individuals achieve their personal recovery goals. Case management is typically a once-monthly service.  

Qualifications for Services

To qualify for TCM, individuals must complete a Comprehensive Evaluation to determine eligibility, be a recipient of a Prepaid Medical Assistance Program (PMAP) and meet the requirements for Minnesota Rule 79 case management services. They must also live within Benton, Stearns, Sherburne, or Wright County.

Services are available to individuals with Medical Assistance (MA) or utilizing CMMHC’s Sliding Fee Scale Discount Program.

Dawn completed her bachelor’s degree in social work from St. Cloud State University and her master’s degree in mental health counseling from Walden University. She is a licensed professional Clinical Counselor and a licensed Social Worker. Dawn has been in the human services field since 1997 before joining the CMMHC team in 2007. She is currently an Outpatient Therapist in our Waite Park office and the Clinical Supervisor for the Adult Targeted Case Management Team (TCM) and Adult Rehabilitative Mental Health Services (ARMHS).

Mary has a master’s degree in Community Counseling from St. Cloud State University. Before joining the CMMHC team in 1994, she was a Family Therapist at Gerard In-Home Treatment Program. She is currently the Early Intervention Family Services Clinical Supervisor, providing consultation and training to other community providers and agencies.

Stephanie completed her bachelor’s degree in Psychology and her master’s degree in Rehabilitation and Addiction Counseling at St. Cloud State University. Her work experience includes working with domestic violence survivors and individuals with disabilities. She joined the CMMHC team in 2015 and is currently the Program Manager for the Adult Targeted Case Management Team (TCM) and Adult Rehabilitative Mental Health Services (ARMHS).
